About the discharge summary
The discharge summary form allows you to:

Capture medications the patient needs to take home

Create discharge letters for the GP and for the patient to take home
The discharge summary form includes a workflow that guides you through the discharge process. During the discharge process, you can prescribe medications for a patient to take home, called TTO To Take Out. Each medication added to the discharge letter can be called a TTO. Also, the list of medications on the discharge letter can be called the TTO. medications.
The contents of the discharge summary form depends on your organisation.
About the discharge summary on a mobile
You can't do this on a mobile.
About the discharge summary on a desktop

You can start a new discharge summary from Inpatients > Patient Detail > Case Notes.

At the top of the discharge summary form, flags outline the stages of the discharge summary workflow.

The discharge summary form includes fields for you to complete. Mandatory fields are marked .

At the bottom of the discharge summary form, there are buttons. The buttons allow you to progress the form to the next stage. You can only select buttons if:

You have permission to do so. For example, you can only mark TTO prescriptions as approved if your role includes the right pharmacy permissions.

All mandatory fields are complete.

At the top of the discharge summary form, flags outline the stages of the discharge summary workflow. When a stage is complete, the flag shows a tick. The current stage is highlighted in blue.
Complete form |
The discharge form has been opened but is incomplete. |
Pharmacy approval |
The discharge form has been sent to the pharmacy, but the TTO prescriptions are not yet approved. |
Dispense TTO |
This stage is when TTO medications are dispensed. This changes to Pharmacy Dispense, Non-pharmacy, or Ward Dispense. |
Pharmacy Dispense |
The TTO list has been approved and is waiting to be dispensed from the pharmacy dispensary. |
Being dispensed |
The pharmacy dispensary has started to dispense the medications. |
Nurse check |
The medications have been dispensed ready for the nurse to confirm are present and correct. |
Non-pharmacy |
The TTO list has been approved and is waiting to be dispensed from a location that is not the pharmacy dispensary. For example, a satellite pharmacy. |
Ward Dispense |
The TTO list has been approved. If there are medications on the TTO list, they are supplied from ward stock. |
Send letter |
The medications have been checked by the nurse, and the discharge letter is ready to be sent. |

At the bottom of the discharge summary form are buttons that you can select.
The text on the buttons depends on your organisation, but the function remains the same.
You can only select these buttons if your role includes permissions to do so, and any mandatory fields are complete.
Send to Pharmacy |
Select if there are TTO prescriptions that must be approved by a pharmacist before they can be dispensed in a pharmacy location. |
Approved, dispense in pharmacy |
Select to approve TTO prescriptions for dispensing in the pharmacy dispensary. |
Approve, dispense other |
Select to approve TTO prescriptions for dispensing in a location that is not the pharmacy dispensary, for example, a satellite pharmacy. |
Pharmacy rejected |
Select to reject TTO prescriptions. This doesn't inform any users that the TTO prescriptions are rejected. |
Dispense started |
Select to record that you have started dispensing the TTO medications. Depending on your organisation, this option appears. |
Undo dispense started |
Select to record that you have not started dispensing TTO medications. Depending on your organisation, this option appears. |
Confirm dispensed |
Select to confirm the approved TTO medications are dispensed. |
Confirm ward received and verified |
Select to confirm that the TTO medications have arrived on the ward and are correct. |
Cancel discharge |
Select to cancel the discharge form. This doesn't prevent any previous elements of the workflow from progressing. For example, dispensing medication. If the TTO list has already been sent to pharmacy, refer to your organisation’s policies and procedures. |
Recall, TTO changed |
Select to recall TTO prescriptions before they are approved by the pharmacy. |
Approve, ward supplied TTO |
Select if there are no TTO prescriptions included in the discharge form. Or, select to approve TTO prescriptions that are supplied from ward stock and not from the pharmacy. |
Recall, TTO changed after approval |
Select to recall TTO prescriptions that are already approved by a pharmacist. |
Complete, send discharge letter |
Select this to generate, print, and digitally send the discharge form. |
